Jason, It's on SCCA.com in the File Cabinet under "Insurance". I don't know if everyone has read access rights to it, or if it's restricted in some way. I'll email a copy of the pdf to you, JIC. Also, I printed a few copies of this newer style form a number of months ago and put them in the trailer....
